How to Get Rid of Pearly Penile Papules

Four methods actually remove pearly penile papules: electrodesiccation, CO2 laser, cryotherapy, and surgical excision. All four work the same way, by physically destroying the tissue. Nothing applied to the surface can remove a papule, because a papule is solid fibrotic tissue rather than an infection or a blocked pore. That rules out every cream, oil and toothpaste method online, and we tested the popular ones ourselves before concluding it.

Having it done at a clinic costs $800 to $3,000, and no insurer anywhere will cover it. The same electrodesiccation method performed at home with the PPP-KIT™ costs around $129.

What We Tried, and What Happened

Most pages on this subject list treatments without having tried a single one. Here's what we found before we built our own kit:

The cream taught us something worth passing on. If you want to know whether a beauty product actually works, check whether the seller can still take card payments. The gel we tested can't be bought from its own website any more, because the card companies cut them off after too many customers demanded their money back. It's a test that works on anything in this category, and it costs you nothing to run before you buy.

Since 2020 we've helped more than 25,000 men do this at home. That's where the numbers further down come from — how many sessions men actually need, and what actually goes wrong. Those aren't guesses.

Electrodesiccation (Hyfrecation)

This is what most dermatologists pick for PPP. A micro-electrode delivers radiofrequency energy that creates a controlled arc, destroying the papule's proteins and cell membranes and dehydrating it instantly.

You get numbing cream first, and the whole thing takes 15 to 25 minutes depending on how many papules you have. The spark only works on the surface, so it can't burn deep. The skin takes one to three weeks to heal.

CO2 Laser

The laser heats the water inside each papule until it burns away. It works, and the results are permanent.

The catch is that lasers go deeper by design. If the machine isn't set up properly, the heat travels further into the skin than it needs to, which raises the chance of scarring compared with electrosurgery. It's also the priciest option, because the machines cost a fortune to buy and run.

Cryotherapy (Freezing)

Liquid nitrogen is sprayed on for 15 to 20 seconds. Ice forms inside the papule, the blood supply stops, and it dies off and drops away over the following days. You'd need sessions a month or two apart. It isn't used much for PPP any more, because cold is hard to aim precisely on something this small, and it can leave permanent pale patches on the skin.

Excision and Curettage

The papules are cut or scraped off, sometimes finished with the electrical tool to clear the root. It works, but it carries the highest scarring risk of the four, so it's rarely the first thing suggested for something you're only having done for how it looks.


Doing It at Home

Doing it yourself uses the same electrical method, just turned down to a lower power. What you're saving is the doctor's time, not cutting a corner on the method.

What Actually Goes Wrong

From more than 25,000 customers, in order of how often it happens:

  1. Not going deep enough, so they grow back. Easily the most common problem. First-timers are careful, understandably, so the root survives and the papule is back within a few weeks. That's why most men plan on a second session rather than treating one as a failure.
  2. The numbing wearing off halfway through. Numbing cream lasts about 15 to 30 minutes. If you've got a lot of papules you'll run out partway and have to put more on and wait again. Plan for it.
  3. A skin reaction from too much healing ointment. If you keep using the antibiotic ointment for the full ten days instead of the few days the guide tells you to, your skin can react to it. Stick to the timing.
  4. An allergic reaction to one of the creams. Rare, but it happens — usually to the numbing cream or the healing ointment. You'd get some redness and swelling, and it settles down.

What These Kits Are Not For

They're made for pearly penile papules and Tyson glands. They are not for Fordyce spots on the shaft — those sit deeper in the skin and are far more likely to scar. If you're not sure which you've got, check the identification guide or see a dermatologist first.

What Doesn't Work

One fact explains all of these at once. A papule is an angiofibroma, solid collagen and blood vessels anchored to the glans. It is not a pimple (no pus or sebum), not a wart (no HPV), and not an infection. So there's simply nothing there for a cream to work on.

  • Toothpaste — dries out the skin around them. Drying solid tissue doesn't dissolve it.
  • Tea tree oil — harsh on skin this thin. Using it neat, over and over, risks burns and loss of feeling.
  • Castor oil — it's a moisturiser. You can't moisturise away solid tissue.
  • Removal creams — most are ordinary lotions with a new label on them; we've seen beard balm repackaged and sold as a PPP treatment. A cream strong enough to dissolve solid tissue would have to be an acid. And since papules sit in rings all the way around the head, you'd be spreading that acid over the whole area — which is how men end up with burns.
  • Popping, cutting or picking — papules have blood vessels in them and are rooted into the skin. You'll get bleeding, possibly an infection, possibly a scar, and the root survives anyway.

Healing

After electrosurgery, whether at a clinic or at home:

  • Days 1 - 2 — small scabs form, a bit tender, some weeping. All normal.
  • Days 3 - 5 — scabs start lifting. You'll see the difference.
  • Days 6 - 8 — most scabs gone, skin still pink.
  • Day 10 onward — healed for most men. Any leftover redness fades over the next few weeks.

Laser and cutting take longer, up to two or three weeks. Pale patches from freezing can take months to even out. Keep the area clean, follow the aftercare timing, and don't have sex until it's healed — reopening the area is the single most common reason healing drags on.


Will They Come Back?

Once a papule is destroyed right down to the root, it's gone for good. When men say they came back, it's nearly always one of two things: the root wasn't fully destroyed and it grew again, or a new papule appeared nearby.

Realistically: 60% clear everything in one session, 40% need a second, and men who go carefully the first time sometimes need a third. Plan on two.

Should You Remove Them at All?

Worth saying now that you know the options: PPP is harmless. Somewhere between 14% and 48% of men have them, more often if you're uncircumcised. You can't catch them, you can't pass them on, they've got nothing to do with hygiene, and they usually get less noticeable as you get older. No dermatologist will tell you that you need this done. Every method on this page is about how it looks, and nothing else.

That matters for one reason in particular. If what's really bothering you is how a partner might react, most people have never heard of PPP and will take their cue from you. One sentence — "they're normal, they're harmless, you can't catch them" — settles it for most people, at no cost and no risk. How to have that conversation.

Deciding to remove them anyway is completely fair enough. Most of our customers got there after years of explaining themselves. Just make it a decision, rather than a reaction.


How to Choose

  • You want a doctor to do it and money isn't the issue → electrosurgery or laser. Book a consultation.

Common Questions

With the numbing cream on properly, most men say it's a slight tingle and nothing more. The cream lasts about 15 to 30 minutes, so if you've got a lot of papules you'll need to put more on partway through. Being a bit tender for a few hours afterwards is normal.

Almost never. Papules are harmless, so removal counts as cosmetic — which leaves it outside private insurance, Medicare and the NHS alike. More on insurance and PPP removal.

Surface methods only touch the papule itself and don't reach the nerves underneath, and men who've had it done generally report no change. Cutting them out goes deeper, which is one more reason to stick to the surface methods.

Anywhere from a few to several full rows around the head. The number doesn't mean anything medically.
